          INTRODUCTION

          The Assam Cancer Care Foundation (Community Outreach Program) in Diphu has become
          a beacon of hope for many, embodying the theme “United by Unique.” By integrating medical expertise
          with community-based interventions, the team has showcased the profound impact of personalized care.

          One notable case is that of Jahanara Sarkar, a 53-year-old woman diagnosed with oral cancer. The Diphu
          team adopted a holistic approach to her care, combining medical treatment with emotional and logistical
          support. From coordinating family involvement to leveraging resources for funding, the District Coordinator,
          Patient Navigator, doctors, and nurses worked as a cohesive unit to address her needs. This collaborative effort
          highlights the importance of an integrated care model, inspiring communities and healthcare professionals
          alike to continue their fight against cancer.

          Jahanara Sarkar lived a modest life in Diphu, Karbi Anglong, with her daughter. Despite having two sons
          residing nearby, they chose to live separately and offered her no support. For years, Jahanara ignored small but
